Late Renewal

If your registration was not renewed for the current biennial renewal period, you can renew by completing the late renewal application and returning it to the Board with the renewal fee and penalty ($137.50). The current renewal period began on November 1, 2005 and ends on October 31, 2007. Only licenses which expired on October 31, 2005 can use the late renewal form. Licenses which expired prior to November 1, 2003 must apply for reinstatement.
